- About the role
There is a 0.8 FTE perm role available for a senior nurse within the South/West
Community Service team. We are looking for dynamic, clinically sound,
pro-active nurses with great leadership skills and a - big picture - outlook.
A large component of this role is an approach to client intervention
plans that promotes maximal health status and independence for the client. You
will be delivering care in client - s homes, so a clean, manual driver - s license
is a must.

As part of our high functioning team you - ll enjoy case management,
providing safe and effective clinical assessment, nursing care and education to
older people with complex needs within the Older Persons Health and
Rehabilitation Community Service. You will have expert clinical skills and a
background in Older Persons Health/Gerontology.
You - ll be a confident, autonomous practitioner with the ability to
demonstrate professional accountability within the scope of practice and have
