**About the Role**

Hauora a Toi - Bay of Plenty is seeking a dynamic Registered Health Professional (either a Registered Nurse or Allied Health professional) to lead and implement the START (Support Transfer and Accelerated Rehabilitation Team) service based at Tauranga hospital. START is an innovative early discharge and rapid response team designed to facilitate the safe and timely return of older people to their homes after hospital stays.

As the START Coordinator, you will oversee the delivery of individualized care plans provided by trained Health Care Assistants or support workers. Your leadership will drive the team’s success in achieving its core goals, which include improving functional independence, reducing hospital readmissions, and ensuring seamless integration into primary care. This senior role offers the opportunity to foster collaboration, build resilience, and promote a culture of excellence within a high-performing team.

**About the Team/Service/ Location**

The START team is a multidisciplinary group of dedicated professionals committed to enhancing patient outcomes by providing rapid and effective support to older people transitioning from hospital to home. **About you**
- Proven experience as a Nurse Coordinator, Leader, or Manager.
- Exceptional communication skills to support patients, whānau, and staff.
- Strong relationship-building abilities with internal and external stakeholders.
- Passion for fostering team collaboration and developing individual potential.
- Commitment to operational safety and evidence-based practices.
- Experience in change management to enhance and refine care models (preferred).
- Understanding of business ethics and leadership principles.
- Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
