**Te Tūranga Mahi - About the role**:
The Pae Ora Facilitators work in partnership as part of the Pae Ora Whānau Care team to help facilitate an inclusive approach to providing services and opportunities to whānau across Te Pae Hauora o Ruahine o Tararua.
The Pae Ora Facilitators are integral for enabling culturally responsive care for whānau delivered by Māori for Māori. By providing support and advocacy for whānau accessing secondary care and primary health services. These roles are part of the Pae Ora whānau care team responsible for lifting the level of culturally relevant care. Facilitating, assisting, and supporting seamless pathways for whānau from primary care, through hospital services and back into the community.
This role takes primary responsibility for first point of contact and whānau engagement. It will take overall responsibility to ensure that the whānau wellbeing journey is tracked and that their experience and outcome reflects sound whānau ora practice.

**Ngā Pūkenga, Ngā Tohu ōu - What you will need**
- Relevant and practical experience working with whānau
- Knowledge of the local community providers and support
- A current full driver’s licence
- Knowledge of te ao Māori me ōna tikanga and Te reo Māori
- A drive to be solution focused and think outside of the square
- Understanding that from a Māori world view, we are collective and holistic in our approach and response when working alongside whānau
- Understanding of the whānau ora framework, encouraging whānau to determine how they want/need to be supported in their journey to wellbeing
