Mental Health Triage Clinician

**Ko **tāuake mahi | What you will be **doing**
You will undertake the initial phone screening and assessment of patients referred to the Pegasus Primary Mental Health Service to ascertain the best pathway of care, in conjunction with General Practice Teams, Mental Health Clinicians and/or external service providers. You will:
- Identify key issues of the referred patient via phone assessments and define what is required. The information will be used to assist in forming a plan for treatment.
- A resource for patients and General Practice to gain knowledge about and establish contact with community agencies and support services.
- Where there are more serious mental health concerns or risk factors, liaising with the patients General Practitioner as to a referral to and from psychiatric services.
- All work is carried out within the guidelines of the documented triage pathway
- The role will also have active involvement in service planning/development initiatives.
This job is across Monday to Friday, and with future potential to support evening clinics.

The Pegasus Mental Health Team works in collaboration with General Practice teams, the patient, family, whānau and the wider community, to provide the best pathway of care for patients with psychological difficulties and/or mental illness.

- Mental health experience, including care co-ordination and a relevant professional registration under the HPCA Act
- Highly developed interpersonal and communication skills
- Ability to develop relationships and work collaboratively with a wide variety of people
- High level of cultural competence with an understanding of (or active interest in) and commitment to Te Tiriti o Waitangi principles and biculturalism.
- Established relationships or ability to build relationships within Maori, Pasifika and wider community networks
- The ability to work autonomously and as part of the wider team
- Enthusiasm and passion about improving healthcare
