Location: Gisborne Hospital

This service has been recently established to work in collaboration with partner organisations to enable older people to receive care centred around their needs. The service is committed to maintaining the best possible health and greatest level of independence for older people with avoidance of unnecessary hospital stays a key objective.

In this role the successful applicant will deliver Home and Community Support services to clients who have recently been discharged from hospital or who have experienced a significant change in their health circumstances. This support may include personal cares (hygiene cares) and to a lesser extent home help (housework), with a focus on recovery and rehabilitation. The work may be scheduled across seven days, so can include weekends and early evenings.

Te Ara Tiaki Pakeke service promotes equity and sensitivity to different cultures and is responsive to the voice of pakeke and whānau.

- Ability to follow complex instruction
- Ability to engage with different team members, patients and whānau
