**Care Capacity Demand Management Coordinator (CCDM)**

**Permanent, part time**

**56 hours per fortnight (0.7 FTE)**

An exciting opportunity has arisen to work within Te Whatau Ora CCDM Programme with a small but high achieving team. The focus of this role is to provide operational leadership and facilitation to develop, drive and sustain the CCDM Programme within Te Whatau Ora Hawke’s Bay.

We are looking for a Registered Nurse who can work in partnership, shows ability to connect, engage and influence multiple stakeholders, from the clinical units to governance, as it is through others that you will achieve positive outcomes.

Objectives include but are not limited to:

- Prioritise a focus on patient safety and quality improvements in relation to care and processes within the nursing service;
- Coordinate and implement best practice tools and guidelines;
- Develop and sustain constructive relationships that enhance connections and ability to influence;
- Lead and collaborate on programme development, communication and implementation, and feedback loops to ensure ongoing engagement across the board.
