**About the role**

An exciting opportunity has become available to work as a **Needs Assessor Service Coordinator** and join our passionate and multi-disciplinary team at Lakes NASC based either in Rotorua or Taupo.

Our work is fast paced and dynamic. We offer a positive and supportive team environment which is responsive to clients’ needs as well as enabling professional opportunities for learning and development.

The role of NASC is to complete holistic assessments of the needs of our clients, and coordinate support services for older adults with health and disability related needs, and some younger people whose needs relate to a chronic long term health condition. We also work with clients who have recently discharged from hospital, as well as palliative care clients. Our aim is to support our clients to continue to live safely at home, or enable them to move into Residential Care if needed.

This role involves working with clients across the Lakes region (Rotorua, Mangakino, Taupo, Turangi).

**About you**

You should have
- New Zealand Registered Social Worker, Occupational Therapist, Physiotherapist or Nurse.
- Current New Zealand APC in appropriate scope
- Allied Health or Nursing qualification and relevant professional experience
- Current NZ Annual Practicing Certificate
- Full NZ Driver’s License

You will have proven assessment and clinical reasoning skills. In addition, you will have experience of working in partnership with clients and their whanau to meet their support needs. You will have excellent communication skills, as well as a record of positive relationship building.
