Assessment and Support Coordinator

As an Assessment and Support Coordinator with Te Whatu Ora - Health New Zealand Lakes, you will be responsible for coordinating support services for older adults with health and disability-related needs, as well as some younger people with chronic long-term health conditions.
About the Job- Key responsibilities include completing holistic assessments of client needs, developing and implementing individualized support plans, and collaborating with healthcare professionals to ensure seamless care coordination.
- We require a strong understanding of assessment and clinical reasoning skills, excellent communication abilities, and a commitment to building positive relationships with clients and their whanau.
- A Full NZ Driver's License is essential for this role.
