The Role
We're seeking an experienced Care Coordination Specialist to oversee the rostering of complex care clients and their support teams. This rewarding role allows you to build strong relationships with complex care support teams and clients, while participating in Support Worker training and ensuring adherence to sector standards.
Key Responsibilities
- Oversee the rostering of complex care clients and their support teams.
- Participate in Support Worker training, ensuring adherence to sector standards.
- Build strong relationships with complex care support teams and clients.
- Assist with training of the complex care teams.
- Identify and support recruitment needs of the identified complex care clients.
Requirements
- Experienced in long term conditions, aged residential care or medical care.
- Intermediate level computer and on-line information recording skills.
- The ability to plan and organise caseloads effectively, to establish priorities and meet deadlines in a demanding, client-focused environment.
- A current full driver's license is essential.
