Your Key Responsibilities:
- Support individuals in achieving their recovery and overall well-being goals
- Assist with day-to-day activities, including accessing social and recreational groups, education and employment, budgeting, and liaising with clinical teams and other community providers
- Develop and implement personalized support plans

Requirements:
- A current full driver's licence
