**About the Role:**

You will be responsible for delivering high-quality patient care, providing a full range of primary care services in partnership with the Central Clinical Team. You will also support our HCA team & Primary Care Nursing team, providing direction & delegation in the provision of high-quality patient care.

**Requirements:**

  • Current registration
  • Have a real desire to work in primary care long term
  • Excited by change & a champion for model of care improvements
  • Strong interpersonal, communication, analytical, and organisational skills
  • Ability to work under pressure, exhibiting strong problem-solving skills
