The Role
">We are seeking a skilled Registered Nurse to join our Community Mobile team. As a key member of our team, you will forge meaningful connections with individuals and families, fostering well-being through skilled assessment, thoughtful care, and integrated working with Primary and Secondary care partners.
Your Responsibilities
">You will use your nursing knowledge and skills to assess, plan, implement, monitor, and evaluate tangata whai ora/taiohi/whanau health needs. You will provide dynamic and holistic nursing care to achieve best health outcomes, engage and work alongside community support workers, tangata whai ora, and whanau to improve knowledge of illness, self-management, and prevention of complications and promote well-being.
About You
">To be successful in this role, you will have a minimum of 2 years' experience in Nursing and mental health, an understanding of the New Zealand context of service delivery, a current New Zealand Health registration in Nursing, and a current full New Zealand driver's license. You will also possess knowledge, skills, and experience in developing care plans, risk management plans, supporting other clinicians, and working with a multi-disciplinary team.
