We are seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will work closely with tāngata whaikaha and their whānau to ensure they receive the support and care they need.

Key Responsibilities:

  • Conduct assessments and re-assessments to identify needs and develop personalized care plans
  • Work collaboratively with tāngata whaikaha and whānau to coordinate services and support
  • Foster strong relationships with community partners and stakeholders
  • Stay up-to-date with relevant legislation and best practices in intellectual disability services

Requirements:

  • Previous experience working with the disability community and frontline client care
  • Proven knowledge of relevant legislation, including the Intellectual Disability (Compulsory Care and Rehabilitation) Act 2003 and The Criminal Procedure (Mentally Impaired Persons) Act 2003
  • Strong relationship management skills and ability to communicate effectively with diverse stakeholders
  • Able to work independently and as part of a team
