We are seeking a skilled and compassionate Respiratory Care Specialist to join our Community Nursing team at Omni-Health Pte Ltd. As a key member of our team, you will provide dedicated nursing care to individuals with long-term respiratory conditions.
About the Role- Home and Hospital Visits: Provide expert nursing care to patients with respiratory conditions in their homes and at the hospital.
- Collaboration: Work closely with General Practice teams, Secondary Care Specialists, and other healthcare providers to ensure integrated care.
- Self-Management Support: Empower patients and their families through self-management strategies for respiratory health.
- Whanau Ora Approach: Develop personalized care plans with patients and their whanau, promoting independence and long-term health.
- Assess patient needs and develop individualized care plans.
- Provide education and support to patients and their families on respiratory health management.
- Collaborate with multidisciplinary teams to coordinate care and ensure seamless transitions.
- Communicate effectively with patients, whanau, and healthcare professionals to promote cultural awareness and patient-centered care.
