Community Healthcare Specialist

We are seeking a highly skilled Community Healthcare Specialist to join our team at WOFO. As a District Nurse, you will play a vital role in delivering care to patients in their own homes.
About the RoleIn this rewarding position, you will travel within the community to assess, monitor, and support patient care, helping patients maintain health and independence at home.
Key Responsibilities:
- Provide clinical care and support to patients in their homes
- Coordinate with doctors, pharmacists, and other professionals to manage patient care
- Educate patients and families on health management and preventive care
You will be working closely with our experienced team to ensure that patients receive high-quality care and support. Your expertise and compassion will make a real difference in the lives of our patients and their families.
RequirementsTo succeed in this role, you will need:
- Nursing qualification with current registration
- Experience in community nursing or similar roles
- Strong organizational and time-management skills
You will be working in a mobile role that offers flexibility and independence, allowing you to set your own schedule. Our supportive team is dedicated to helping you succeed in your role and providing continuous support every step of the way.
