The Role
We are seeking an experienced Support Worker to join our Health and Wellbeing Team, supporting clients across the Tāmaki Makaurau region. Key responsibilities include:
- Developing and implementing individualised support plans for clients.
- Collaborating with the team to design and deliver group programs that promote healthy lifestyles.
- Providing one-on-one support and guidance to clients, addressing their physical, emotional, and spiritual needs.
- Maintaining accurate records and reports to ensure seamless communication and continuity of care.
Essential Requirements
- New Zealand Certificate in Health and Wellbeing (NZCHW) or equivalent qualification.
- Proven experience working as a support worker, preferably in mental health and addiction services.
- Strong understanding of Te Ao Māori principles and practices.
- Excellent communication and interpersonal skills.
- Able to work independently and as part of a dynamic team.
