Job Summary
We are seeking a motivated Health and Wellbeing Coordinator to join our team. In this role, you will be responsible for coordinating and delivering health and wellbeing services to individuals with intellectual disabilities, promoting their overall health and wellbeing.


Key Responsibilities
* Coordinate and deliver health and wellbeing services, including medical appointments and therapy sessions.* Develop and implement personalized health and wellbeing plans, ensuring the best possible outcomes for each client.* Collaborate with colleagues and other stakeholders to ensure seamless care delivery and coordination.* Utilize your expertise to promote positive relationships and de-escalate situations.


Requirements
* Strong background in health and wellbeing, preferably with experience working with individuals with intellectual disabilities.* Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients and colleagues alike.* Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.* Valid Full NZ driver's license.
