As a Lifestyle Change Specialist, you will work across the lifespan, supporting individuals of all age groups.

You will collaborate with and serve as a resource person for the general practice team, demonstrating strong cultural awareness to effectively support our diverse community. You will work on the GP premises, in the Pathways office, or make community visits.

**Requirements:**

  • Eligible to work in NZ
  • New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognized equivalent
  • Satisfactory police check
