Job Description:

We are seeking a highly skilled and dedicated professional to join our team as a Renal Disease Specialist. In this role, you will be responsible for the administration of the renal retardation programme which provides disease-specific case management for patients with chronic kidney disease (CKD Stage 1 to 5).

Responsibilities:

  • Assess, plan, implement, and evaluate care of patients with renal disease
  • Conduct teaching and counseling for patients and their families/carers
  • Conduct/specialty nurse-led clinic for patients and their families where needed
  • Co-ordinate patients' access and treatment sequence where required
  • Assist with overall coordination of renal replacement therapy
  • Assist with placement of patients in dialysis facilities as required
  • Deliver patient care (inclusive of education and counseling) according to established standards of care and practice using patient care plan/clinical pathways
  • Contribute to multidisciplinary patient management
  • Apply understanding of prevention of medico-legal issues in relation to patient care
  • Collaborate and co-operate with physicians and other healthcare providers in the delivery of patient care and services
  • Demonstrate proper use and care of equipment
  • Demonstrate proper and economical usage of consumables and supplies
  • Assist to review and develop patient education materials in consultation with Specialty Nurses, Nurse Managers, Doctors, and Paramedical Staff
  • Assist to review, develop, and evaluate clinical pathways and/or other plans of care in consultation with Specialty Nurses, Nurse Managers, Doctors, and Paramedical Staff
  • Precept newly joined renal coordinators in delivery of specialty care where needed
  • Participate in quality improvement activities
  • Take on additional responsibilities or tasks as assigned by the Head of Division
  • Maintain a clean and safe working environment
