Renal Disease Specialist

Job Description:
We are seeking a highly skilled and dedicated professional to join our team as a Renal Disease Specialist. In this role, you will be responsible for the administration of the renal retardation programme which provides disease-specific case management for patients with chronic kidney disease (CKD Stage 1 to 5).
Responsibilities:
- Assess, plan, implement, and evaluate care of patients with renal disease
- Conduct teaching and counseling for patients and their families/carers
- Conduct/specialty nurse-led clinic for patients and their families where needed
- Co-ordinate patients' access and treatment sequence where required
- Assist with overall coordination of renal replacement therapy
- Assist with placement of patients in dialysis facilities as required
- Deliver patient care (inclusive of education and counseling) according to established standards of care and practice using patient care plan/clinical pathways
- Contribute to multidisciplinary patient management
- Apply understanding of prevention of medico-legal issues in relation to patient care
- Collaborate and co-operate with physicians and other healthcare providers in the delivery of patient care and services
- Demonstrate proper use and care of equipment
- Demonstrate proper and economical usage of consumables and supplies
- Assist to review and develop patient education materials in consultation with Specialty Nurses, Nurse Managers, Doctors, and Paramedical Staff
- Assist to review, develop, and evaluate clinical pathways and/or other plans of care in consultation with Specialty Nurses, Nurse Managers, Doctors, and Paramedical Staff
- Precept newly joined renal coordinators in delivery of specialty care where needed
- Participate in quality improvement activities
- Take on additional responsibilities or tasks as assigned by the Head of Division
- Maintain a clean and safe working environment
