About the Team
Our team is passionate about delivering high-quality care to our patients. We value kindness, excellence, and collaboration, and we are committed to serving all New Zealanders. We work closely with the Lakes community addiction providers and provide specialist intervention for people to optimise recovery and quality of life as well as case coordination.
- Provide specialist intervention for people to optimise recovery and quality of life as well as case coordination
- Provide community mental health care based on current best practice and collaborative working within the interdisciplinary team
- Ensure all inter-disciplinary referrals within the team are performed with consent from whaiora and their whanau (where applicable)
- Work collaboratively with complementary internal and external partner organisations
- Develop sound legislative knowledge, particularly as pertains to the SACAT Act
About the Model of Care
We are implementing a new model of care, Te Ara Tauwhirotanga: pathways that lead us to act with kindness. This model was co-designed with over 400 participants from the Lakes DHB community and clearly articulates what works for people in their recovery and healing journey.
