We work closely with the Lakes community addiction providers and provide specialist intervention for people to optimise recovery and quality of life as well as case coordination.

  • Provide specialist intervention for people to optimise recovery and quality of life as well as case coordination
  • Provide community mental health care based on current best practice and collaborative working within the interdisciplinary team
  • Ensure all inter-disciplinary referrals within the team are performed with consent from whaiora and their whanau (where applicable)
  • Work collaboratively with complementary internal and external partner organisations
  • Develop sound legislative knowledge, particularly as pertains to the SACAT Act

About the Model of Care
We are implementing a new model of care, Te Ara Tauwhirotanga: pathways that lead us to act with kindness. This model was co-designed with over 400 participants from the Lakes DHB community and clearly articulates what works for people in their recovery and healing journey.
