Job Description
">The Medical Assistant assists providers with ongoing patient monitoring and chronic care management. Key responsibilities include:

- Monitoring patient blood pressure, blood sugar, and other vitals, as well as helping patients with medication adherence and scheduling medical appointments.">
- Communicating with patients on a regular basis to provide education and support, promoting lifestyle changes to improve long-term health outcomes.">
- Maintaining accurate and up-to-date electronic medical records (EMR).">
- Performing other duties as assigned by healthcare providers.">
Required Skills and Qualifications
">To be successful in this role, you must possess the following skills and qualifications:

- A minimum of 5 years of experience as a Medical Assistant or nurse.">
- Passion for working with older adults and excellent customer service skills.">
- Preventative patient care background preferred, with experience in primary care, cardiology, or diabetes clinics highly desired.">
- Ability to work independently and collaboratively as part of a team.">
- Strong organizational and time management skills.">
- Computer skills appropriate to the position, including proficiency in electronic medical records (EMR) systems.">
- Excellent written and verbal communication skills, as well as administrative and clerical skills.">
- Familiarity with medical terminology and concepts.">
