Job Role: As a Community Mental Health Nurse, Occupational Therapist, or Social Worker in our Triage team, you will be responsible for receiving referrals, assessing clients' needs, and coordinating care with other healthcare professionals. You will work closely with other team members to ensure seamless delivery of services and achieve the best possible outcomes for clients.
Responsibilities:
- Receive and assess referrals from various sources, including general practitioners, hospitals, and community organizations
- Determine the level of need and urgency of each referral, prioritizing cases accordingly
- Coordinate care with other healthcare professionals, including nurses, doctors, and social workers
- Provide education and support to clients and their families on mental health issues and available resources
- Maintain accurate records and reports, adhering to confidentiality and data protection policies
Requirements:
- Current annual practicing certificate enabling working in the mental health sector
- Experience working within the mental health field, preferably in a triage or assessment role
- Strong interpersonal skills, therapeutic engagement, critical thinking, and communication skills
- Able to work effectively in a multidisciplinary team and autonomously
- Good understanding of the community and services available
- Knowledge and understanding of Te Tiriti o Waitangi
- A current full driver's license
