Nurse Specialist in Complex Care

About the Role
The NHS Cheshire and Merseyside Integrated Care Board is seeking a highly skilled Nurse Specialist to join our team. As a key member of our commissioning team, you will play a vital role in assessing and coordinating care for adults and young people with complex health needs.
Key Responsibilities
- To manage a complex caseload of adults and young people in transition with complex health needs, ensuring timely and robust ongoing reviews are completed.
- To plan, commission, and procure bespoke care packages for eligible individuals, embedding personalisation into care and support plans.
- To provide ongoing case management, working collaboratively with patients, carers, and professionals across health and social services to ensure seamless care transitions.
Requirements
- A degree or equivalent level of experience in health or social care, with current NMC registration.
- Experience in commissioning adult complex care packages and awareness of the integrated health and social care commissioning agenda.
- Strong leadership skills, with the ability to work collaboratively with stakeholders and implement national policy.
