The successful candidate will be part of the inpatient occupational therapy team, focusing on early assessment and identification of how Occupational Therapy can contribute to ensuring safe discharges through assessment of occupational performance with the aim of reducing hospital readmissions.

This role offers a great opportunity to work closely with the wider multidisciplinary team and to contribute to the delivery of high-quality healthcare services.

**Key Responsibilities:**

- Assess and identify the occupational needs of patients/clients within the hospital setting.

- Develop and implement individualized treatment plans to address occupational performance deficits.

- Collaborate with the multidisciplinary team to ensure seamless care transitions.

- Participate in quality improvement initiatives to enhance patient outcomes.

**Requirements:**

- Registration with the NZ Occupational Therapy Board.

- Clinical experience in a hospital setting.

- Excellent verbal and written communication skills.

- Ability to work well as part of a team.
