About the Role:

The Healthcare Transition Specialist at Parx Home Care plays a critical role in ensuring seamless transitions of care for patients. As a key member of our team, you will be responsible for facilitating effective communication between healthcare providers, patients, and families to promote optimal discharge planning.

Key Responsibilities:

  • Maintain compliance with federal, state, and local regulations, as well as adhere to Agency policies and procedures.
  • Develop and maintain strong relationships with Skilled Nursing Facilities (SNFs) and other healthcare providers to generate home healthcare referrals and ensure smooth transitions of care.
  • Promote Parx Home Care's services to referral sources, educating them about benefits and features.
  • Evaluate patient readiness for discharge and home health care needs through initial contact in SNFs.
  • Collaborate with physicians, discharge planners, and other healthcare providers to align home health care plans with medical goals.
  • Actively obtain and follow up on referrals, ensuring timely submission of necessary documentation for patient admission.
  • Serve as primary point of contact for clients at bedside in SNFs, addressing inquiries, concerns, and feedback in a timely and professional manner.
  • Provide education to patients and families regarding transition process, available services, and expectations post-discharge.
  • Conduct regular follow-ups to ensure continuous business and identify opportunities for upselling or cross-selling additional services.
  • Maintain ongoing communication with parties involved in patient care to resolve issues and ensure smooth transitions of care.
