Job Description
This position involves administering skilled nursing care in the home to clients requiring professional nursing service. The Admission Nurse works closely with the multidisciplinary team to provide comprehensive care and coordination of services.

Responsibilities
The key responsibilities of this role include:

  • Triage emergency phone calls from Home Care patients after hours
  • Making visits as needed
  • Documenting interdisciplinary communication to ensure necessary follow-up with the team during the next business day

Requirements
To succeed in this role, you will need:

  • A valid driver's license in the State of Delaware with no more than 3 violations
  • Prior Home Care nursing experience
  • Clear communication skills, both written and verbal
  • The ability to keep confidential information regarding patients and team members

Qualifications
The ideal candidate will possess:

  • RN licensure
  • BLS certification
  • Knowledge and experience with Electronic Health Records
