We are currently seeking a highly motivated Primary Care Navigator Professional to join our team.
In this role, you will work closely with general practices to support the primary mental health needs of patients.
Your key responsibilities will include:
- Working with patients and their families to identify and address barriers to quality care
- Providing education and support to promote health literacy and self-management
- Collaborating with healthcare professionals to ensure comprehensive care
To be successful in this role, you will need:
- A Certificate in Mental Health and Addictions (L4) or equivalent
- Experience working with people with mental health and addiction issues
- Strong communication and interpersonal skills
