We're looking for a skilled Community Healthcare Navigator to join our team at Access Community Health. As a Community Healthcare Navigator, you'll play a vital role in supporting people to remain active, safe, and independent in their own homes and communities.

About the Role
As a Community Healthcare Navigator, you'll be responsible for overseeing the rostering of complex care clients and their support teams. You'll participate in Support Worker training, ensuring adherence to sector standards, and build strong relationships with complex care support teams and clients.

Key Responsibilities
- Oversee the rostering of complex care clients and their support teams.
- Participate in Support Worker training, ensuring adherence to sector standards.
- Build strong relationships with complex care support teams and clients.
- Assist with training of the complex care teams.
- Identify and support recruitment needs of the identified complex care clients.

Requirements
- Experienced in long term conditions, aged residential care or medical care.
- Intermediate level computer and on-line information recording skills.
- The ability to plan and organise caseloads effectively, to establish priorities and meet deadlines in a demanding, client-focused environment.
- A current full driver's license is essential.
