This role involves working directly with clients, whanau, NGO service providers, Primary Health sector and Aged Residential Care facilities with the support of our wider Inter-Disciplinary team.
This role requires exceptional assessment skills, the ability to develop individual management plans promoting client-centered goals and excellent interpersonal skills.
