We are seeking a skilled Clinical Needs Assessor to join our team at Health New Zealand | Te Whatu Ora. As a Clinical Needs Assessor, you will play a vital role in assessing the needs of older adults and developing personalized support plans.

Your Responsibilities

  • Conduct comprehensive assessments of older adults in their homes and various health settings using the interRAI national assessment tool.
  • Develop client-centered care plans that focus on achieving optimal outcomes and promoting independence.
  • Connect clients with formal and informal support services to enhance their quality of life.

Requirements

  • You must be a Registered Health Professional with a current practising certificate.
  • Prior experience working with older adults in a hospital, community, or aged care environment is essential.
  • You should have an excellent understanding of community resources and services available to older adults.
  • A valid NZ driver's license is also required.
