Job Description:
The North Adult Community Psychiatric Service at Hillmorton Hospital requires a Community Rehabilitation Worker to join their multi-disciplinary team. The successful candidate will provide support to consumers who are disabled by mental illness, enhancing their independence and well-being.
Key Responsibilities:
- Work closely with Case Managers to develop and implement individualized treatment plans.
- Assist consumers in areas of daily living, management, and socialization within the community.
- Contribute to a positive and supportive team environment.
Required Skills and Qualifications:
To be successful in this role, you will have:
- A Certificate in Mental Health studies or other relevant training.
- Experience in Community Mental Health delivery.
- Skills and experience in working within a multi-disciplinary team.
- Commitment to Treaty of Waitangi and bi-cultural practice.
