Job Description:

The North Adult Community Psychiatric Service at Hillmorton Hospital requires a Community Rehabilitation Worker to join their multi-disciplinary team. The successful candidate will provide support to consumers who are disabled by mental illness, enhancing their independence and well-being.

Key Responsibilities:

  • Work closely with Case Managers to develop and implement individualized treatment plans.
  • Assist consumers in areas of daily living, management, and socialization within the community.
  • Contribute to a positive and supportive team environment.

Required Skills and Qualifications:

To be successful in this role, you will have:

  • A Certificate in Mental Health studies or other relevant training.
  • Experience in Community Mental Health delivery.
  • Skills and experience in working within a multi-disciplinary team.
  • Commitment to Treaty of Waitangi and bi-cultural practice.
