Palliative Care Opportunities

We are seeking a Home-Based Healthcare Professional to join our team at Access Community Health. As a key member of our palliative care team, you will work in the community to provide high-quality patient-centered care and support.

Your primary responsibility will be to provide symptom management, end-of-life care, psychosocial support, advocacy, and liaison services to clients and their families. You will also review the effectiveness of our Access service to ensure that we provide safe and effective care and support to our clients.

Key Responsibilities:

  • Provide symptom management and end-of-life care to clients in their homes.
  • Offer psychosocial support and advocacy services to clients and their families.
  • Collaborate with multidisciplinary teams to coordinate client care and ensure seamless transitions between healthcare services.
  • Stay up-to-date with best practices and guidelines in palliative care to ensure that clients receive the highest quality care possible.
