Job Summary

This role is a critical part of our efforts to address homelessness, providing comprehensive case management services to individuals and families in need. As a specialist case manager, you will be responsible for developing and implementing personalized support plans, working closely with clients, landlords, and community partners to ensure seamless coordination of services.

Key Responsibilities
  • Conduct thorough assessments to identify client needs and develop tailored support plans
  • Collaborate with internal teams to provide comprehensive wrap-around services, including mental health and addiction support
  • Develop and maintain strong relationships with clients, landlords, and community stakeholders to ensure effective service delivery
  • Advocate on behalf of clients to address systemic barriers and promote their rights and dignity
Requirements

To succeed in this challenging yet rewarding role, you will require:

  • A degree in Social Work or a related field, with relevant experience working in the mental health and addiction sector
  • Basic understanding of Te Tiriti o Waitangi and its application in social services
  • Excellent communication and interpersonal skills, with the ability to build trust with clients and stakeholders
