Diabetes Clinical Leadership Position

This is an exciting opportunity for a motivated and experienced Diabetes Clinical Nurse Specialist to join our team. The successful candidate will be responsible for working with communities to build strong foundations for health and wellbeing of patients with diabetes.
You will have a deep understanding of the needs of patients and communities, enabling them to exercise authority over their health and wellbeing. Additionally, you will grow and continually develop the Diabetes service and its care delivery to a high standard.
- Main Accountabilities:
- Work with communities to build, maintain, and enable strong foundations for health and wellbeing of patients with diabetes.
- Prioritize disease prevention, health promotion, and health & wellbeing.
- Understand the needs of patients and communities, enabling them to exercise authority over their health and wellbeing.
- Grow and continually develop the Diabetes service and its care delivery to a high standard.
