The Role

We have an exciting opportunity for a registered nurse with a minimum of five years of diabetes nursing experience to join our team as a Clinical Nurse Specialist. This permanent role offers a 0.6 FTE position, Monday to Friday, with hours to be discussed at interview stage.

In this leadership position, you will focus on empowering people to manage their diabetes by working closely with communities to build strong foundations for health and wellbeing. You will prioritize disease prevention, health promotion, and health & wellbeing, understanding the needs of patients and communities, and enabling them to exercise authority over their health and wellbeing.

Your Responsibilities

  • Develop and deliver holistic care to patients with diabetes, prioritizing disease prevention and health promotion.
  • Lead the development of nursing practice to improve health outcomes and management of diabetes.
  • Work collaboratively in a multidisciplinary team to provide medical and diabetes nurse specialist consultation services to hospital inpatients.
  • Contribute to the growth and development of the Diabetes service, ensuring high standards of care delivery.

About the Service

The Diabetes Service provides outpatient consultation services for patients referred from general practitioners and other health professionals. It offers ongoing surveillance of diabetes management and complications status for selected patients who are at higher risk of developing diabetes complications.
