About the Role: As a Care Coordinator, you will be responsible for conducting assessments and re-assessments with individuals with intellectual disabilities and their families. You will identify all funded and non-funded services and support options, and coordinate a package of care that meets individual needs and priorities. This will involve working closely with other stakeholders to ensure seamless delivery of services and support.

You will also provide support to help individuals understand their rights and the process of requesting a review, while maintaining strong relationships with individuals, families, and other stakeholders. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

We are looking for an experienced professional with a proven track record of working with individuals with intellectual disabilities and their families. You should have a deep understanding of relevant legislation and policies, as well as facilitation and problem-solving skills. A specialist knowledge of service coordination would be an advantage, but not essential. Strong relationship management skills and the ability to communicate effectively with a wide range of stakeholders are essential for this role.
