Advanced Practitioner in Palliative Care

We are seeking a compassionate and organised Registered Nurse to join our Community Palliative Care team as a Care Coordinator. In this role, you will be responsible for coordinating the care of patients and their families, ensuring that they receive the best possible support.
As a Care Coordinator, you will work closely with other healthcare professionals to develop individualised care plans, which may include symptom management, pain control, and emotional support.
You will also be responsible for educating and supporting patients and their families, and ensuring that they have access to all necessary resources and services.
Responsibilities- To coordinate patient care and develop comprehensive care plans.
- To provide high-quality palliative care services, including symptom management and pain control.
- To work collaboratively with other healthcare professionals to ensure seamless care delivery.
- To educate and support patients and their families.
To be successful in this role, you will need:
- A current NZ Annual Practising Certificate.
- Minimum 2 years' experience in nursing, preferably in a palliative care setting.
- Excellent communication and interpersonal skills.
- A strong commitment to delivering high-quality patient care.
- Current full NZ Driver's Licence.
