Care Coordinator

About the Role:

We are seeking a skilled Service Coordinator to join our Forensic Coordination Service – Intellectual Disability team.

As a key member of our team, you will work closely with tāngata whaikaha (people with an intellectual disability) and their whānau (family) to provide coordinated support and services.

Your primary focus will be on ensuring that tāngata whaikaha understand their rights and the process of requesting a review. You will also support them and their whānau to hold meaningful expectations and aspirations for their lives.

You will identify all funded and non-funded services and support options for tāngata whaikaha and coordinate a package of care, in consultation with tāngata whaikaha and whānau as appropriate.

Key Responsibilities:

  • Previous experience working with the disability community and frontline client care experience with Intellectual Disability clients is essential.
  • Proven knowledge of relevant legislation such as the Intellectual Disability (Compulsory Care and Rehabilitation) Act 2003 and The Criminal Procedure (Mentally Impaired Persons) Act 2003 is required.
  • Experience using facilitation and problem-solving strategies to work collaboratively with others both internally and externally is necessary.
  • A specialist knowledge and understanding of service coordination would be an advantage but not essential.
  • Strong relationship management skills and the ability to communicate with a wide range of stakeholders is crucial.

This role requires the successful candidate to obtain a satisfactory Children's Worker Check and be willing to travel for work from time to time.
