**The Role**

In this role, you will work closely with tangata whaikaha and their whanau to identify their needs and develop a plan to meet those needs. You will also be responsible for coordinating a package of care that is tailored to their individual requirements.

Key Skills and Qualifications:

  • A specialist knowledge and understanding of service coordination would be an advantage but not essential
  • Strong relationship management skills and the ability to communicate with a wide range of stakeholders
