They are dedicated to ensuring the best care for adults with this condition by providing a range of free services in Health Promotion, Research, Information, and Support.The PositionBe part of a collaborative team, reporting to the Manager Nursing Services, looking after your own territory where you will provide support to home-based patients and their families through diagnosis, treatment, and preparation for life afterwards.Care is drawn from an integrated services approach, supported by the Community Liaison service, Information Service Line, Counselling & Psychology, and Volunteer Service.Conduct community phone and home visits to provide nursing assessment, planning, and support, as well as cover for the Information Service.Maintain information, assist with obtaining new resources, and maintain client records.Internal communication and liaison, eg working with the Nurse Specialist on service delivery & projects.External communication including assistance with education and building positive working relationships with other external service providers.Support a few support groups.YouOngoing professional development training and support will be provided to build your knowledge (adult patients, i.e., 18 years plus).
A self-starter with a Full NZ Driver's License (essential), you will conduct your own triage and enjoy providing practical and clinical information/support to clients in their homes (averaging 3-4 visits/day) - helping them through their treatment and ensuring their care progresses smoothly.A Registered Nurse with a current APC is required; you may be a past or present Community, District, Practice Nurse, or oncology nurse looking to extend your knowledge, or an exceptionally experienced Nurse seeking a lifestyle change, or even a Plunket Nurse if you have additional experience.The Package$46.84/hour paid fortnightly (salary will increase slightly as it's just been approved by the board), FULL use of a company car, Laptop, Printer, Cell phone, paid per hour/internet connection, 4 weeks leave (and your birthday off), 28 hours a week (4 days/week, a 7-hour day).
Insurance membership offer, reimbursement for your registration, and external supervision (4 per year).
