Our Organisation Te Pae Hauora o Ruahine o Tararua MidCentral (MidCentral) is part of Te Whatu Ora, Health New Zealand and provides healthcare services to over 180,000 people across the Manawatu, Horowhenua and Tararua regions. With a large hospital campus in Palmerston North, it also provides key health care and support services to neighbouring communities.
The Opportunity For an Advanced Practitioner Social Worker (PPPR) to join the Post-Acute Community Rehabilitation team. This position is a permanent 40-hour per week position. This is a great opportunity to experience working in the hospital environment and the community.
The Post-Acute Community Rehabilitation Service (PACR) is primarily dedicated to supporting clients immediately post their acute care episode.
The Advanced Practitioner Social Work Role (PPPR) provides safe and clinically effective comprehensive assessment and intervention, demonstrating advanced knowledge and skills to manage complex presentations to patients, clients, and their whanau.
Key responsibilities include:
Ensuring a focus on safe and high-quality patient/client care.Coordinating systems, processes, and procedures in regard to the Protection of Personal and Property Rights Act 1988.Providing expertise and acting as a key resource concerning the Protection of Personal and Property Rights Act 1988.Providing clinical leadership across services to develop the clinical capability of others.Delivering individual and group education to upskill social workers and other health professionals across MidCentral.Providing high-quality social work services to individuals and their families/whanau referred through the Mid Central PACR service.Supporting patients and whanau with the Protection of Personal Property Rights (3PR) process and the transition from inpatient to community.Assisting with patient discharge from health recovery beds and involvement with stroke and non-acute rehabilitation.The successful applicant must have effective oral and interpersonal communication skills, the ability to work under pressure, and a willingness to learn and complete ongoing training. Awareness of principles around person-centered practice, sensitivity to cultural differences, and commitment to the Treaty of Waitangi are essential. A current practicing certificate and driver's license are also required.
