With community hubs in Whakatu (Nelson), Wairau (Blenheim) and Motueka effective in delivering a range of quality health services, from mobile clinics or in the home.Our mission to support whanau to achieve greater hauora, to thrive, underpins everything we do: Ma te huruhuru ka rere te manu - Enable whanau to achieve wellness, knowing that when they need us, we will be there throughout all stages of their life.Mo tenei turanga mahi / About this roleThe purpose of this role is to provide navigation support to whanau wanting to stop smoking, as well as administrative support to the Te Ha (smoking cessation) team/s.
In this part time role, you will spend 20 hours per week on the ground working with whanau and the remaining 10 hours per week providing administration support to the Te Ha team, from either the Whakatu or Motueka community hubs.
Working to ensure the integrated and co-ordinated approach to providing better health and well-being outcomes for whanau.
Helping to develop whanau plans, goal setting and supporting the whole whanau to implement plans that will lead to positive change.Ko wai koe / About youYou are invested in providing and supporting better health outcomes for whanau in the community.
An advocate to reinforce health messages in the wider community developing and delivering the best standards of care using Kaupapa Maori approaches.
