If so then Intermediate Care is the place for you which has a role to offer as a Nurse Specialist
The Intermediate Care Team is part of the Community and Long-term Conditions Directorate, which provides an intensive, short term multidisciplinary service for adult patients both within their own homes and/or Age-Related Residential Care facilities (ARRC). Our aim is to keep patients out of hospital by either preventing them being admitted or assisting to get them out into the community earlier and provide care within the home.

Nga Pūkenga Motuhake | Important Skills
If you are a level 4 nurse with experience in the community, emergency departments or primary health care organisations, then this maybe the role you are looking for. It would be preferable to be working towards or have some relevant post graduate education. The role includes 8-hour rostered shifts covering a 7 day service between the hours of 0730-2200 within the community (Monday to Sunday).

**Being a Nurse Specialist within Intermediate Care involves**:

- Working within a multidisciplinary team of health professionals and having the privilege of working with patients within their own environment.
- Mentoring and leading staff through both direct and indirect patient care.
- Involved in the development of a service and contributes to the development of interdisciplinary standards of practice and evidence-based change.
- Having advanced clinical assessment skills.
- Nursing and Allied health input that supports the transition from hospital to home.
- Supporting our Interim Care patients.
- Supporting the acute care needs of people in the community to help prevent further crisis and/or deterioration in their condition.
- Holistic patient centred care.
- Support the improvement of the integration of community care models.
