The Hospice Palliative Care team at Nurse Maude are dedicated to providing comprehensive and compassionate care to our community. The Nurse Maude Hospice Palliative Care Service provides specialised care and support for patients who have a life-limiting illness using a multidisciplinary approach. Care is centred on the needs of patients and their families/whanau and embraces the physical, social, emotional, and spiritual elements of wellbeing - tinana, whanau, hinengaro and wairua - and aims to enhance quality of life. We work within the Te Whare Tapa Wha model of care.

Role and Responsibilities:
As a Needs Assessor, your primary responsibility and key duties will be to
- Undertake interRAI and support needs level assessments for those affected with life limiting illness who require a change in their living circumstances either for respite or long-term care. Assessments take place either in the patient's own home or in the Hospice inpatient setting.
- have the ability to provide support, advice, and guidance to ensure quality palliative care.
- You will be working within a multi-disciplinary team and develop strong working relationships with key stakeholders in the community. Experience in this field would also be advantageous.

To be successful in this role, you should possess the following qualifications and skills:

- You need to be a registered health professional with a current practising certificate
- Desire to work with clients and their whanau with palliative care needs,
- Your communication skills will be outstanding as you navigate the best care options in partnership with patients and whanau.
- A full driver's licence
