The Pasifika Regional Coordination Hub (PaRCH) provides a Pasifika-led, end to end process for all Pacific peoples and their family in the Northern region. The PaRCH aims to coordinate and provide the public health, clinical health and social/welfare needs to Pacific family in a timely, appropriate and safe way in response to an urgent need for the Pacific population.

The PaRCH Social Worker will act as a key member of the Pasifika Regional Coordination Hub (PaRCH) team and will provide a high standard of comprehensive Social Work services to individual, their families and the community. Using your social work expertise, you will deliver a culturally excellent case and contact management response, emergency response, support Outreach teams, and other areas under the Pacific Public Health team.

This role is based at Greenlane Clinical Centre.

**Nga Pūkenga Motuhake | Important Skills**
- NZ registered Social Worker with a current practising certificate
- Minimum of 5 years clinical experience
- Previous case management experience in a Health setting
- Fluent in a Pacific language is an advantage
- Have a strong understanding of equity approaches to care
- Have great communication and critical thinking skills
- Have a good understanding of health, disability and complex conditions and services
- Full, clean drivers licence
