**Kōrero mō te tūranga - About the role**

We are excited to offer a new opportunity for a registered social worker / kaimahi toko i te ora to join our specialist team of social workers and psychologists in supporting people and their whānau following a new or recurrent diagnosis of cancer. This role is worked primarily in outpatient and community settings across Capital, Coast and Hutt Valley district, with a focus on Porirua and Kāpiti. The social worker will have a focus on reducing barriers to accessing supports for people with cancer and creating essential links between agencies supporting people with cancer in this locality. This is a part time role (0.6FTE/24 hours per week) and there is flexibility with how these hours are worked across the week.

The clinical aspect of this role is worked primarily in outpatient and community settings with people and whānau following a diagnosis of cancer. The social worker will provide comprehensive psychological and social needs assessments, therapeutic social work interventions, and coordinate support services for people and whānau following their diagnosis and through treatment.

The social worker in this role will work collaboratively as part of the local community to ensure that people with cancer have their psychological, social and supportive care needs met. They will work alongside local providers such as the Cancer Society, Iwi and Māori health care providers and other local services, with a focus on streamlined, client and whānau focussed care.

Duties will include:

- Working with clients diagnosed with cancer, and their whānau to improve their ability to cope with the difficulties of a new or recurrent diagnosis, as well as their ability to engage in cancer treatment
- Developing and maintaining key relationships with local stakeholders involved in the provision of supportive care for cancer patients and their whānau
- Completing comprehensive psychological and social needs assessments, risk assessments and financial hardship assessments with people who are referred to the service
- Working with the team to promote and grow our services across the region and in the community

**Mō tō mātou rōpū - About our team**

You will be joining a friendly, dynamic and highly motivated Allied Health Oncology team, including psychologists and social workers who are dedicated to improving health outcomes for those living with cancer. Our small, but very passionate team, work across Capital, Coast and Hutt Valley district.

We value collaborative working and support other professions and services to grow their skills and capabilities in the provision or psychological and social support, as well as providing direct support to people diagnosed with cancer and their whānau.

Our service is flexible in the way we deliver care to clients, and is continuously looking at ways to improve our reach to people at risk of poor outcomes from cancer.

**Moū - About you**

You will have a real passion for working in this area, and be highly motivated to help us provide a quality service. You will be able to work collaboratively and effectively as part of a multi-disciplinary team and across services to ensure the best possible outcomes are achieved. You will have experience in the health sector and an understanding of grief, loss and change as it relates to long term health conditions. You will have an understanding of the impact of long term and chronic conditions on holistic wellbeing, and a commitment to improving outcomes for our Māori and Pacific communities.

The successful applicant will have the following skills and attributes:

- NZ Registered Social Worker with current annual practising certificate
- Minimum of 3 years social work practice, preferably with experience in a health setting
- The ability to complete comprehensive psychological and social needs assessment
- Navigating and working within the legal requirements of health care and social work
- A clear understanding of the health needs of Māori and Pacific Island communities
- Therapeutic social work counselling experience (desirable)
- Previous work in a health setting and good understanding of health social work (desirable)
