He Kōrero mō te tūranga - About the Role
This role is situated within the Community Allied health team in the Western Bay of Plenty, a region encompassing many different communities, covering a wide geographical area. The team is split into three geographical locality teams with the majority of staff working within an assigned area. Each locality has a lead who is responsible for managing the day to day operations of the locality.
Over the last two years, the team has undergone significant changes in order to meet the needs of the community. Several new services have been put in place to ensure the team is responsive to those who experience a decline in their wellbeing, and to ensure that the service delivers enabling and equitable intervention that promotes flourishing. Some of these new services include:
- Enablement programmes from the community or hospital, delivered in the home.
- Transitional care rehabilitation for those unable to directly return home following illness or injury
- Rapid response, for urgent needs that require a same day/next day response to prevent admission to hospital or facilitate a timely transitional out of hospital.
- Experienced intervention for those with long term chronic health and disability.
- Allied health service embedded within a kaupapa māori haurora to improve access to enablement programmes for māori. The team supports clients/ urihaumate and whānau with their enablement goals; utilising person-directed approaches to improve wellbeing, guided by “what matters to me”.
The Community Allied health team is made up exceptional clinicians who care about their community and amaze us every day with their expertise, commitment and innovative thinking. As the team lead, you will be continuing to lead the team through the development of these new services, and mentoring the locality leads to lead well within their assigned locality. It will be your role to monitor overall service data, identify trends and inequities and work with others to ensure that the team development aligns with strategic direction of the organisation. You will be expected to form strong working relationships with a range of stakeholders both inside and outside of Te Whatu Ora.
He Korero mōu - About You
This is an exciting and important role in the development of one of our key community services. As such, we are looking for an experienced leader with at least five years clinical experience who cares about the community, our staff and the difference allied health can make. You will have led a team before and have had experience in implementing service developments to improve service delivery. It is essential to have an active awareness of equity for Māori and to be able to monitor and develop service to resolve any inequities that exist. You will be a natural communicator with the ability to build authentic relationships and foster collaborations with key community partners.
