We have an exciting opportunity for a registered Social Worker to join our Pacific Health Development team.

The role also includes engaging, supporting, facilitating and working with other organisations to achieve the best outcomes for the community. We are looking for someone that can provide a high standard of comprehensive Social Work services to individuals, their families and the community. You will be responsible for providing a comprehensive social work service within the context of the interdisciplinary team, for patients and their families with health-related issues under Fanau Ola.

The role will require you to deliver the service across Counties Manukau District services and in the community. To meet the needs of Pasifika Patients and Fanau, some rostered and after-hours shifts may be required. The role will support the Team leader and be a resource for the wider team. You will carry a caseload of Pasifika Fanau with complex needs.

**Essential**:

- Registered with NZ SWRB and hold a current Annual Practicing Certificate
- Preferred extensive experience in working with Pasifika Patients and Fanau
- Full and clean driver’s licence
- Ability to speak a Pacific language will be advantageous, but not essential
