We are a highly motivated team of specialist Children’s Community Nurses providing a paediatric nursing service for a variety of babies, children and young people. This encompasses working in the locality and integration models being implemented in Counties Manukau District. We help families care for their children and young people discharged from hospital following acute and long-term illnesses/disabilities. This includes palliative care and transitional care for high-risk neonates.

In this role as a Community Health Worker, you will be contributing to the efficient operation of the service by managing clinical supplies and consumables for patients in the service (ordering, packing, distributing and delivering across the district), and providing hands on support for the clinical team in a variety of ways.

Counties Manukau has a commitment to the ongoing development of working in partnership with its communities to improve health status of all, with particular emphasis on Maaori and Pacific peoples and other communities with health disparities.

**Essential**:

- Minimum 1-year experience in the health environment
- Experience working with children and families
- Full, clean NZ driver’s licence (including the ability to drive a van)
- Excellent communication skills
- Competent with navigating a variety of IT systems e.g. Microsoft Office and health software
- Strong cultural awareness of the diverse cultural groups in South Auckland
- Commitment to the Treaty of Waitangi
- A commitment to and passion for community-based care
- A whanau centred care approach
- Able to work independently in a multi-disciplinary team
