This is an exciting new position piloting a new model of care to support mothers and children affected by diabetes in pregnancy.

The Navigator role will provide wrap-around support to mothers and children affected by diabetes in pregnancy, to assist the best access to the health and social-care they need. You will establish and maintain a strong relationship with mothers, whaanau, wider community and community service providers. You will be an advocate for service consumers by supporting mothers, children and whaanau to establish and maintain a relationship with their community primary and any specialist secondary health care providers.

Furthermore, the Navigator will assist in the provision of individual antenatal / postnatal care plans, and participate in the evaluation of this pilot intervention, relating to antenatal / postnatal care for mothers and their babies / children affected by diabetes in pregnancy.

We will offer full training and support, a friendly diverse team. If you have good advocacy skills, like working as part of a team and, want to put Te Tiriti o Waitangi into action, this could be the job for you.

**Essential**:

- Relevant education (e.g., background in nutrition education / maternal care / diabetes / community health)
- Experience working in a community-based health / social / care environment
- Full, clean NZ drivers Licence
